What is CAQH?


CAQH (Council for Affordable Quality Healthcare) is an online provider data-collection platform. It streamlines provider credentialing by using a standardized electronic profile that meets the requirements of most health plans, hospitals, and healthcare organizations.


How do I create a CAQH?

You’ll need to provide information such as your NPI, license details, and practice information. Be sure to attest to your profile after completing it so that payers can access your data.


How do I find my CAQH ID?


If you’re unsure of your CAQH ID, you can retrieve it using CAQH’s lookup tool:


Enter your identifying information (such as name and NPI), and the system will guide you to your CAQH ID.
