Credentialing providers through Zaya

Getting started: Adding your initial providers

When you join Zaya, we’ll need your full provider information so we can credential each provider (verify qualifications and practice details) and then enroll them with the insurance carriers your practice has opted in to.


How to get started

  1. Download the Practice Intake Form

  2. Complete the full form

    • Facility details

    • Provider details (name, NPI, license type, start date, location)

  3. Email your completed form to credentialing@zayacare.com.

  4. Respond promptly to follow-ups:
    Our credentialing team may email you if additional documents, clarifications or actions are needed. Quick responses help keep your credentialing on track.


A note on timeliness:
We hold ourselves to a high standard for moving credentialing forward quickly, but we also need your collaboration to keep the process smooth. If we don’t receive timely responses from your credentialing contact or providers, and all requested credentialing steps are not completed within 30 days of our initial credentialing kickoff, this will trigger an additional credentialing fee per provider.


Making updates later on

Once your practice is live on Zaya, you may need to update your provider info - for example:

  • A new provider joins your practice

  • A provider leaves your practice

  • A provider changes location, license, or other key details

Why updates matter

  • Claims accuracy: Only enrolled providers can bill in-network.

  • Compliance: Carriers require and regularly audit us on accurate rosters.

  • Patient access: Insurance directories rely on up-to-date information.

  • Faster payments: Avoids denials or delays from incorrect in-network status.

How to make updates to your provider info

  1. Download the same Practice Intake Form

  2. In the “Provider Details” section, specify in the dropdown whether you're looking to "Add" or "Remove" a provider.

    • Adding: fill out the entire row

    • Removing: Provider first and last name and NPI only (no need to fill out the other fields)

  3. Email the form to credentialing@zayacare.com


Credentialing timelines

Because Zaya is a delegated entity with most insurance carriers, we can process your credentialing and enrollment updates much faster than carrier-direct submissions.

  • Our part: We submit updates quickly once we have all required information.

  • Your part: you respond quickly to any requests from our credentialing team

  • Public directories: Carriers control these, and updates may take up to ~3 months to appear, even when enrollment is already active.
