This guide explains which insurance payers you can bill through Zaya, what services and states are covered, and where to be extra careful with plan details.
It’s meant to give you a quick reference and avoid surprises when checking patient eligibility and billing claims. If in doubt, always confirm the patient's coverage through the Zaya portal.


Note, your practice must be opted in and fully enrolled for the resp. insurance through Zaya; meaning the provider has received confirmed in-network status by Zaya’s credentialing team.

Some payers have subplans not covered under our contracts, and plan lineups change often. Below is the list of included subplans we know about. To check a patient, match the payer name and network/program to the table, then confirm eligibility in Zaya’s portal.

Known excluded subplans & programs


We know about a few definite exclusions, but we do not have a complete or permanent list, because payers frequently change their plan lineups.

  • For all payers listed above, Zaya does not cover Medicaid plans. If a patient’s card says “Medicaid,” “Managed Medicaid,” or “Medi-Cal,” we cannot bill it — even if the payer name matches one from the table
  • Zaya does not cover Traditional Medicare, sometimes also called "Straight Medicare" or "Federal Medicare"
  • For UHC & Oxford, Zaya does not cover "The Empire Plan" and "Oxford Liberty Plans"
