How to Set Up Surrogacy Access in CMS for Zaya

Instructions for providers who require full Medicare enrollment and must grant surrogate access to Zaya Credentialing Team.


Why You Need Surrogacy Access:

If you are not yet enrolled in Medicare for your state, Zaya needs access to your information to prepare and submit your enrollment application.


Steps to Set Up Surrogacy Access:

  1. Visit the CMS Identity and Access Management site:  https://nppes.cms.hhs.gov/IAWeb/warning.do 

  2. Click Accept and log in using your PECOS credentials.

  3. Complete the multi-factor authentication.

  4. Click on the My Staff tab.

  5. Select Add Staff.

Enter the following team member information to grant access:


Stacy Sapir

Email: stacy@zayacare.com

Role: User Access (Select PECOS)


Sarah Cevallos

Email: sarahc@zayacare.com

Role: User Access (Select PECOS)


  1. Submit your request.

  2. Zaya Credentialing team will finalize the connection once received.


What Happens Next:

Once surrogate access is established, Zaya will prepare your full Medicare enrollment application. You will then be notified when your electronic signature is needed.
