Getting your group practice started with Zaya Care

  1. Contract Signing

    • You receive a contract from a Zaya Care Team member via Docusign. We ask that you sign the contract in a timely manner.  


  1. Onboarding / Credentialing

    • Zaya will need to gather key information for each of your providers. The following information includes all that will be needed for each provider within your practice:

    • SSN, DOB, and mailing address 

      • License type, state, number, expiration

      • Board certification information, if applicable

      • DEA information, if applicable

      • NPI

      • Education History (including any education gaps, if applicable)

      • Specialties

      • Hospital affiliations, if applicable

      • Employment history (including any work gaps, if applicable)

      • Disclosures

      • Professional liability insurance

      • CAQH login

      • CAQH Attestation form

  2. Once you’ve acquired the necessary information, you may submit the credentialing contact here


  1. Each provider will then be submitted for credentialing with each insurance payer Zaya works with; this means your providers will become in-network with Zaya Care, and are able to see insured clients with billings and claim management handled by Zaya.


  1. As you wait to receive INN status with our partnered insurance groups, we will need for you to submit information for the provider profile. The provider profile is the first point of contact that any potential patient will have when considering booking a session with your provider. The following information includes all that will be needed for each provider within your practice:

    • The provider’s preferred email address 

    • The state(s) the provider’s currently licensed in

    • The degrees or certifications the provider holds

    • What languages the provider can speak in addition to English

    • What modality of care the provider is planning on seeing clients in (Telehealth, Home Visits, and Personal Office)

      • Address of personal office or home address the provider will be traveling from (if applicable)

    • The provider’s mailing address

    • A bio of 200-300 worlds

    • The provider’s approach to care

    • Provider’s specialty & specialty areas 

    • A headshot


  1. Once you’ve collected the information for the provider profile, you may submit the information here


  1. Now, you can sit back and relax. Zaya will handle the rest. Our team will diligently work towards getting your team credentialed and on our platform. If there are any outstanding items or questions, we will reach out to you. Otherwise, Zaya is here to help. 



How group payments work with Zaya

Group practices will have payments funneled into the group owner’s bank account—we leave the smaller details of what you would pay them up to you.


Adding or removing providers on Zaya

Please contact us with a 30-day notice if you wish to remove a provider from our platform. We will take the necessary steps to properly remove them from the platform. 


On the other hand, if you wish to add a new provider from your practice to our platform, please contact us and fill out the previous steps and we will work to get them credentialed and their provider profile completed.
