Welcome to Zaya’s onboarding and credentialing process!
The Zaya Care team is ecstatic to have you begin the journey of becoming part of our network. As you navigate through our credentialing and onboarding process, you will be one step closer to supporting our mission of changing the maternal healthcare space in the United States.
In order to create an easeful transition into our network, we have created a checklist for you to prepare the necessary documents and information that are required for each step of this process. If you have any questions at all, please don’t hesitate to reach out to support@zayacare.com.
CAQH Profile
A CAQH is an online provider-collection solution. It streamlines provider data collection using a standard electronic form that meets the needs of nearly every health plan, hospital, and other healthcare organization.

- NPI (National Provider Identifier)
An NPI is a 10-digit number that will be used to identify you to your healthcare partners, including all payers in all HIPAA standard transactions. An NPI is required, regardless of provider type.

Proof of PLI
Professional liability insurance also called professional indemnity insurance is a form of liability insurance that helps protect professional advice -and service-providing individuals and companies from bearing the full cost of defending against a negligence claim made by a client, and damages awarded in such a civil lawsuit. The coverage focuses on alleged failure to perform on the part of, financial loss caused by, and error or omission in the service or product sold by the policyholder.
The required PLI coverage to be within Zaya Care's network is coverage of 1 million per occurrence and 3 million per individual

- Education History (dates (month & year) & location)
- Employment &/or Education gaps (dates (month & year) & reasoning)
- Date of board certification (if applicable)
Board certification is an option for medical providers who have chosen to become experts within their specialty and to demonstrate their expertise of specific skills and knowledge. - Date of DEA certificate (if applicable)
A federal Drug Enforcement Administration (DEA) certificate is required for all qualified practitioners who write prescriptions for controlled substances. - State licenses (activation dates & expiration dates; if applicable)
IBCLCs do not need to have state licenses to be within our network, regardless of where they are located.

Provider Profile Form
Your provider profile is the first point of contact for patients when seeking the type of care that you provide. This is an opportunity to make an excellent first impression and to market your services. You will receive a link to your profile once it has been created.
Standard contact information (name, pronouns, email)
Modality of care you are planning on providing to clients (personal office, telehealth, home visit)
The physical address of your personal office (if applicable)
The address you will be traveling from when conducting home visits (if applicable)
Your mailing address
A bio about your education, history, work experiences, and who you are as a person and provider (250-350 words)

Specialty areas for your field
