As a part of the onboarding and credentialing process, we ask that you provide your most recent resume/CV.
Your resume should include the following information:
Employment History (dates (month & year) & location)
Education History (dates (month & year) & location)
Employment &/or Education gaps (dates (month & year) & reasoning)
Date of board certification (if applicable)
Board certification is an option for medical providers who have chosen to become experts within their specialty and to demonstrate their expertise of specific skills and knowledge.
Date of DEA certificate (if applicable)
A federal Drug Enforcement Administration (DEA) certificate is required for all qualified practitioners who write prescriptions for controlled substances.
State licenses (activation dates & expiration dates; if applicable)
IBCLCs do not need to have state licenses to be within our network, regardless of where they are located.
