Upon onboarding, Zaya collects your practice's bank account details through an ACH Authorization Form. The information listed on the said form is used by Zaya to send your practice's weekly payments. Should you wish to make any changes to the initial information that you have provided, please follow these steps:

  1. Send an email to support@zayacare.com officially requesting to change your practice's bank account details.

  2. For security verification purposes, Zaya will reach out directly to the practice's decision maker using the phone number we have on file.

  3. Once confirmed, please fill out the ACH Authorization Form via Docusign that Zaya will be sending directly to your email in the soonest possible time.

  4. After processing your request, you will receive an email update from Zaya confirming that the changes have already been reflected.
