This article explains how to add providers to Zaya, make updates after launch, and keep your provider roster accurate for billing and compliance.
Getting started: adding your initial providers
When you join Zaya, we need complete provider and practice information so we can credential providers and enroll them with the insurance plans your practice has selected.
To get started:
Download the Practice Intake Form
Complete all required sections, including:
Facility details
Provider details (name, NPI, license type, start date, location)
Email the completed form to credentialing@zayacare.com
After submission, the Credentialing team may follow up if additional documents, clarifications, or provider actions are needed. Prompt responses help keep the process moving.
Timeliness and fees
Zaya works quickly to move credentialing forward, but your participation is required. If requested credentialing steps are not completed within 30 days of the initial kickoff, an additional credentialing fee may apply per provider.
Making updates after you’re live
Once your practice is live on Zaya, you must notify us of any provider changes, including:
Adding a new provider
Removing a provider
Updates to a provider’s location, license, or other key details
Keeping provider information current is important for:
Accurate claims and billing
Compliance with carrier audits
Correct insurance directory listings
Avoiding payment delays or denials
How to submit provider updates
To make changes:
Download the Provider Update Form (click here) (do not try to edit the original)
In the Provider Details section, select whether you are adding or removing a provider
Adding: complete the full row
Removing: provide the provider’s first name, last name, and NPI only
Email the completed form to credentialing@zayacare.com
