Patient-Facing Transparency Requirements

Modified on Thu, 16 Oct at 4:04 PM

When your practice bills through Zaya’s affiliated professional corporation (PC), all patient-facing materials must clearly identify the PC as the billing entity.

This ensures patients understand who their bills come from and keeps your practice compliant with payer and regulatory requirements.

What’s Required

For all patients billed through Zaya, your materials (digital or paper) — such as:

  • intake or registration forms
  • consent to treat forms
  • invoices and receipts
  • appointment reminders or portal messages

must include a brief statement that identifies the PC as the billing entity.

Approved Text to Include

Use the appropriate statement based on the state where the service is provided:

For New York Patients

“Services may be billed under Bienetre Medical, P.C., the professional corporation affiliated with Zaya Care that manages insurance billing on behalf of your provider.”

For New Jersey Patients

“Services may be billed under Bonheur Medical, P.C., the professional corporation affiliated with Zaya Care that manages insurance billing on behalf of your provider.”

Where to Add This Text

Include the statement:

  • at the bottom of your intake or consent forms,
  • in the footer of invoices or receipts, and/or
  • within EHR or appointment communications if they mention billing or payment.
