These patient-facing materials include:

  • Intake or registration forms

  • Consent to treat forms

  • Invoices and receipts

  • Appointment reminders or portal messages

Use the appropriate statement based on the state where the service is provided:


For New York Patients

“Services may be billed under Bienetre Medical, P.C., the professional corporation affiliated with Zaya Care that manages insurance billing on behalf of your provider.”


For New Jersey Patients
“Services may be billed under Bonheur Medical, P.C., the professional corporation affiliated with Zaya Care that manages insurance billing on behalf of your provider.”


Text Placement

This statement can be included on either one of the following locations:

  • At the bottom of intake or consent forms

  • In the footer of invoices or receipts

  • Within EHR or appointment communications that mention billing or payment
